Causes of Back Pain

Back pain can be caused by many different things. It may come from an acute injury, excessive physical load, a weak core, poor posture, or improper use of the surrounding muscles to support yourself. 

There is not one single answer for every patient. In many cases, posture and how the body is used can contribute to back pain.

How Do We Diagnose the Root Cause of Back Pain?

When a patient comes in with back pain, the first step is a good history. We want to understand how the pain started, how long it has been there, what makes it worse, and what symptoms the patient is feeling.

After that, we run tests and evaluations. Depending on which tests are positive, we can narrow down the diagnosis. Sometimes we do a nerve traction test to see if the nerves are being compromised. If a nerve is compressed, we try to follow it to determine whether the compression is due to a disc or another source.

Not every patient needs imaging. There are times when we send a patient for an X-ray or MRI, depending on the severity of the case and what we find during the exam.

Back Pain Treatment Plan

For lower back pain, treatment usually starts with a warm-up. We use heat and electrical stimulation on the area we are going to work on to increase blood flow.

After that, we do specific myofascial therapy, or muscle work, to loosen muscles that are overworking and very tight.  A lot of pain can come from muscles, so we try to relax those muscles.

We also check the alignment and motion of the spine. If the joints are not moving correctly, we may do an adjustment. After the adjustment, depending on where the patient is in the treatment plan, we may show exercises that help keep the body in a better position. These are exercises patients can do at home to help the process move forward faster.

Disc-Related Back Pain Treatment

If someone has a disc issue, spinal decompression is often highly recommended. The rest of the treatment plan may stay the same, but decompression becomes part of the care.

After the patient is warmed up and treated, we make them lie on the decompression table. The table slowly pulls the lower back in a specific way, allowing the disc material to rehydrate and helping take pressure off the nerve.

The decompression machine has changed the way we treat low back pain and disc pain. It can help people weakened by back pain recover faster. In the past, some patients have taken three to four weeks to start getting out of that painful stage. With decompression, most patients start improving within one to two weeks, and sometimes even within one week.

What Happens If Back Pain Is Not Treated

The long-term consequence of not treating back pain can be future disability. That may mean lower back surgery, stiffness, ongoing pain, or not being able to move around with your kids or grandkids. It can also affect daily life by making it harder to stay active, bend, lift, or move comfortably.

Back Pain Patient Case Studies

Severe Disc-Related Back Pain Recovery with Spinal Decompression

Chief complaint: A patient who was in his late 30s had severe back pain. He had a three-year-old child and needed to lift the child in and out of a car seat. However, he could barely move. He came into the office in tears, partly because of the emotional impact of the pain.

Diagnosis: Based on the tests, he was diagnosed with a herniated disc. An MRI confirmed that it was a severe disc issue. He could not even take long strides because any longer step would pull on the nerve.

Treatment: He was started on spinal decompression immediately. Muscle work was done around the back to loosen the joints and the area around the pain. Since he was in an acute state, very light forms of adjustment were used.

Outcome: Within about 10 to 15 minutes on the decompression table, he reported the pain had already decreased. While he was on the table, he felt about 50 percent better. Within three or four visits, he was about 60 percent better. Within 12 sessions, he was about 90 percent better. Before the end of the treatment plan, he was feeling 100 percent better and did not feel like he needed the last few sessions.
