Causes of Neck Pain

Neck pain is similar to low back pain, but with the neck, we do not have the whole body weight on it.

When neck pain is not caused by trauma, it is usually related to our posture at work or an underlying issue that is adding stress to the spine. Some people have poor posture where their head is tilted or forward. In that position, the neck is taking the weight of the head at a difficult angle.

That forward or tilted posture can drive force downward into the spine and cause compression.

Based on the patient’s history, we try to figure out what is causing the posture. Sometimes it may be related to a lack of mobility in the rib cage or the way the patient sits and moves during the day.

How Do We Diagnose Neck Pain

When a patient comes in with neck pain, we do a full exam and check the range of motion of the neck.

We also do specific exams that apply controlled stress to specific areas of the neck to see where the pain is actually coming from. We palpate and touch the body to feel the muscles and joints. A lot of times, one side may feel very tight compared with the other side.

The diagnosis is based on the exam. We also do a nerve traction test in the upper body to see if a nerve is being compressed.

Neck Pain Treatment Plan

Treating neck pain is much like treating low back pain. We work on the muscles around the neck and also focus on the middle back.

For neck pain, middle back treatment is very important because when the middle back is stiff, the neck has to do too much. In many cases, neck pain is really a neck and middle back issue.

We loosen up the muscles and adjust the joints that are sticky, tight, and not moving correctly.

Depending on the exam, if the patient also has a disc herniation or disc bulge, we may use the decompression table. If the neck is compressed and the discs need to be opened up, decompression may be part of the treatment.

If it is not a disc issue, treatment may include soft tissue work, postural realignment exercises, and adjustments.

What Happens If Neck Pain Is Not Treated

If neck pain is not treated, it can eventually get worse.

It can get to the point where a person is not able to move their neck as much as they want. They may start to rely on moving the whole body, especially while driving, cooking, looking at a loved one, or watching TV.

Neck pain can continue to fester and become worse over time. Sometimes it eventually ends up needing surgery. Surgeries can have side effects, and they do not always help in the long run.

Stiff Neck Management with Chiropractic Care

Chief complaint: A patient came to our clinic with an extremely stiff neck. The patient could not turn the head well and had a lot of anxiety while driving because they could not turn enough to see blind spots.

Diagnosis: During the assessment, we found that poor posture was making the patient overuse one side of the body. The muscles on that side had become very tight, which made it difficult for the patient to turn the neck in one direction. 

Treatment: Care included loosening the muscles, trigger point therapy, myofascial release, and adjustments to open up the fixations in the cervical spine. The patient also had headaches, so trigger point work was done around the suboccipital area (base of the skull). Posture and ergonomics were also changed.

Outcome: With the combination of treatment and posture changes, the patient no longer had anxiety from driving, had much less pain, could turn the head without pain, and was able to do the activities they loved.
